The links below are to forms, instructions and documents needed for you to complete an application for Commissioning or Ordination. Begin by reading the memorandum & checklist.  Look through the requirements and mark all deadlines on your calendar. Make sure you choose the correct instructions for your process. Some forms have two completion options from which to choose.  You only need to complete a form using one of the available options.

Notes:

• A Final Seminary/COS Transcript showing graduation with M.Div. (confirm if in current permanent file) is required to be in your file before Annual Conference begins.
• INTERVIEWS WITH FULL BOARD FEBRUARY 2-3, 2026 in Myrtle Beach. The specific location will be sent to you at a later date.
• Letter to candidates from BOM Registrar setting date and time of interview approximately 3-4 weeks prior to meeting.
• FINAL APPROVAL AT ANNUAL CONFERENCE June 7-10, 2026 at The Greenville Convention Center, Greenville, SC: Must be approved by 3⁄4 majority vote of Clergy Session; Reception and Ordination Service at Annual Conference
